Most U.S. Men Skip Pre-Biopsy MRI for Prostate Cancer

- National Comprehensive Cancer Network has strongly recommended pre-biopsy MRI since 2020, and the test is the standard of care in Europe, Canada, Australia, and the U.K., yet MRIs were used ahead of biopsies in only about a third of U.S. cases in 2022.
- Mark Emberton of University College London called the U.S. gap "one of the world's worst global scandals," noting that 30–50% of patients could avoid biopsy entirely with a prior MRI scan.
- Systematic biopsies — the dominant U.S. approach — detect cancer in only about 50% of cases when it is present and carry infection risks as high as 7%, while up to 70% of newly diagnosed prostate cancers are low-grade and need no treatment.
- American Urological Association Quality Registry data show that 50% or more of U.S. men with clinically insignificant prostate cancer still receive treatment, despite the lesions having a near-zero chance of metastasizing.
- The 2018 PRECISION trial demonstrated that MRI-first protocols detect more clinically significant cancers and fewer insignificant ones than systematic biopsy alone, findings that have not translated into widespread U.S. practice.
- U.S. guidelines diverge from European ones by recommending a systematic biopsy alongside any targeted biopsy when an MRI finds a lesion, a practice critics like Gilbert Welch of Brigham and Women's Hospital say drives overdiagnosis by looking harder than is warranted in asymptomatic men.
Why it matters: With more than 333,000 new prostate cancer cases annually in the U.S. and a 99% five-year survival rate, the choice between MRI-first and systematic biopsy determines whether hundreds of thousands of men are subjected to unnecessary treatment of harmless tumors — a decision currently driven less by evidence than by slow technology adoption, urologists' MRI-reading inexperience, and lost biopsy income.
