Personalized Plans Boost Dementia Cognition 13.7 Points in Trial

- Kat Toups at Bay Area Wellness led a trial of 73 people (average age 65) with mild cognitive impairment or early-stage dementia, assigning 50 to personalized plans targeting infections, hormonal deficiencies, and nutritional gaps, while 23 continued usual care.
- The personalized group also followed a plant-rich diet, exercised six days a week, did daily cognitive training games, and received sleep and stress guidance; after nine months their overall cognitive score rose 13.7 points versus a 4.5-point decline in the control group on the CNS Vital Signs test.
- Memory scores in the treatment group improved 10.6 points (vs. a 2.7-point decline), executive function rose 9.8 points (vs. down 2.2), and processing speed climbed 6.9 points (vs. down 1), with Toups reporting statistically significant improvements in over 90% of patients.
- Blood biomarkers of Alzheimer's and signs of dementia on brain scans did not change in either group, and independent imaging scientist Andrew Surmak cautioned that cognitive gains may reflect functional improvement rather than modification of underlying neurodegenerative pathology.
- Experts including Ana Daugherty (Wayne State) and Christin Glorioso (NeuroAge Therapeutics) called the results promising but said it's unclear how much benefit came from the targeted medical interventions versus the lifestyle changes, and called for larger, blinded, placebo-controlled trials.
- Toups said the 23 control-group participants who were later offered six months of the same interventions did not improve as quickly as those who started earlier, arguing 'there's no time to waste when your brain is degenerating'; the study is posted as a preprint on Preprints.org.
Why it matters: If confirmed in larger trials, this precision-medicine approach could shift early dementia care from symptom management toward treating the specific infections, hormonal deficits, and nutritional gaps driving decline in each patient. The 73-person sample and unchanged biomarkers mean it is not yet practice-changing, but it offers the most rigorous evidence to date that a multi-pronged personalized plan can outperform standard care on cognitive tests.
