- Extern will shut its Royal Avenue needle exchange on 2 November and replace it with a "significantly reduced street-based service" after losing its city-centre premises.
- Extern has searched for more than a year with help from Belfast City Council, the Public Health Agency, the Department of Health and landlords, but no replacement site has been found; Royal Avenue is being redeveloped under Belfast Stories.
- Mal Byrne, Extern’s director of harm reduction and mental health services, said the closure leaves people who inject heroin or cocaine without a place for clean needles, safe disposal, wound care and advice on healthcare and temporary accommodation.
- Extern said almost 20,000 used needles were safely returned to its Royal Avenue premises last year, while its outreach team removed more than 13,000 discarded needles from Belfast streets—roughly 20% more than in previous years.
- Cocaine use is believed to be rising in Belfast city centre, according to Extern, which estimates 1,000–1,500 people at any one time are experiencing difficulties with drug use.
- Public Health Agency recorded 32,667 Enhanced Needle and Syringe Exchange Service transactions in Northern Ireland in 2024–25, up 7.4%, and about 721,000 needles and syringes distributed, up 7.5%.
- One service user said the exchange was a place to chat and build relationships with staff, describing Extern’s workers as "like family" and warning that closure would have a big impact on their mental health.
Why it matters: Extern’s 2 November closure suspends a fixed-site service that returned nearly 20,000 used needles last year and provided sterile equipment, healthcare and housing advice. For the 1,000–1,500 people Extern estimates are experiencing drug difficulties in Belfast, support will shift to a significantly smaller street operation, with discarded-needle removals already roughly 20% above previous years.
