Beef-Liver Iron Supplements: No Trials, Heavy Metal Risk

- Beef-liver supplements are marketed as more effective and gentler than standard iron tablets, but no clinical trials have tested whether their natural haem iron raises iron levels better than ferrous sulphate or ferrous fumarate.
- Mamadou Bah at the London School of Hygiene & Tropical Medicine ran a 12-week trial giving roughly 200 iron-deficient babies in the Gambia beef-blood haem iron versus ferrous sulphate; anaemia dropped from about 85% to 45% in both groups, with no advantage for haem iron.
- Heba Mostafa El-Ghareb at Mansoura University tested 300 US-imported raw beef livers from marketplaces in Mansoura, Egypt, and found most contained mercury, lead and arsenic exceeding international permissible limits; no studies have tested whether beef-liver supplements carry similar contamination.
- Some studies have found haem-iron supplements cause less constipation and diarrhoea than non-haem iron tablets, a potential benefit for people who cannot tolerate traditional supplements.
- Research has linked high haem-iron consumption to elevated risk of bowel cancer and heart disease, adding a further caveat to the trend.
- Beef liver is rich in vitamin A and B12, and the 1934 Nobel prize in medicine honoured US doctors who discovered pernicious anaemia (a B12-deficiency condition) could be treated with daily beef liver.
Why it matters: For the roughly one in four people worldwide estimated to be anaemic — especially those who cannot tolerate the constipation and gut side effects of standard iron tablets — beef-liver supplements offer a pricier alternative whose claims of superior iron absorption remain unproven in clinical trials, while carrying unresolved heavy-metal contamination risk from cattle livers exposed to polluted feed and water.
