- Dronabinol eliminated PTSD nightmares entirely in more than one-third of patients after ten weeks, with another 21% reporting their nightmare burden was at least halved.
- Stefan Röpke and colleagues at Charité - Universitätsmedizin Berlin ran the double-blind, placebo-controlled trial with more than 170 PTSD patients, comparing cannabis-derived THC drops against a cannabis-flavored placebo taken each evening.
- Nightmare severity dropped by 3.7 points on a 0-to-8 scale in the Dronabinol group versus 2.2 points in the placebo arm, and roughly five out of six Dronabinol patients said their health had markedly improved.
- No severe side effects or withdrawal symptoms were detected; mild-to-moderate dizziness, headaches, and increased appetite were more frequent among those taking Dronabinol.
- The treatment acted specifically on nightmares and sleep — researchers could not show it reduced overall PTSD severity or accompanying depression.
- Germany has no medication approved specifically for trauma-related nightmares, leaving the Nature Medicine findings to address a documented treatment gap.
Why it matters: For the more than 170 trial participants who endured recurring trauma nightmares — a symptom with no approved medication in Germany — Dronabinol delivered meaningful relief without severe side effects or withdrawal, but the drug's specificity to nightmares rather than overall PTSD means it addresses one stubborn symptom, not the full disorder.
