- Rutgers researchers published a study in Annals of Internal Medicine linking GLP-1 drugs (Ozempic, Wegovy, Mounjaro, Zepbound) to a modest increase in ischemic optic neuropathy, a rare condition where reduced blood flow to the optic nerve can cause sudden, potentially permanent vision loss.
- Lead author Chintan Dave said the absolute risk increase is small — approximately three to four additional cases per 10,000 patients treated with GLP-1s over 18 months — but clinically significant because the resulting vision loss is often permanent.
- The study analyzed a large database of U.S. adults ages 18 to 65 with Type 2 diabetes who had recently started medications including glucagon-like peptide-1 receptor agonists, focusing on patients new to treatment.
- Cases of the eye condition were more common among men and adults in the older portion of the study population (ages 50 to 65), prompting researchers to flag that group for prompt assessment of sudden vision changes.
- Non-arteritic anterior ischemic optic neuropathy accounts for more than 80% of cases; it usually affects part of the vision in one eye rather than causing total blindness, and only about one-third of patients experience any improvement, typically within six months.
- The researchers cautioned that the findings show an association, not causation — differences in patients' underlying health could explain the elevated risk, and further research is needed to determine whether the medications themselves drive the condition.
Why it matters: With millions now taking GLP-1 drugs for diabetes and weight loss, even a small absolute risk of a vision-robbing condition matters: roughly 3–4 additional cases of irreversible optic nerve damage per 10,000 patients treated over 18 months, concentrated in men and people aged 50–65 who are already the heaviest users.
