- Charité - Universitätsmedizin Berlin ran phase one of the COMPARE trial — the first clinical study of CD19 CAR T-cell therapy in rheumatoid arthritis — enrolling six patients (three women, three men, ages 31–69) who had each failed up to eight prior biologic or targeted therapies.
- All six participants showed marked reductions in disease activity over up to one year of follow-up, and three achieved sustained medication-free remission — though one of those three later relapsed and the remaining three did not achieve a complete response.
- CAR T cells tracked down disease-driving B cells in the bone marrow, lymph nodes and joint tissue, not just in the bloodstream, and autoantibody levels dropped sharply across the group.
- When the B-cell system recovered, the cells that returned were mostly naïve B cells that had never encountered the disease; disease-directed B cells were no longer detectable in nearly all patients, while antibodies from prior vaccinations such as chickenpox and tetanus were largely preserved.
- Safety findings were encouraging: only temporary, mild-to-moderate cytokine release syndrome occurred in all six participants, with no severe neurological complications, no serious adverse events and rare infections.
- Phase 2 of the COMPARE trial will enroll ten additional patients and compare CD19 CAR T-cell therapy head-to-head against an already approved B-cell-targeting rheumatoid arthritis drug; the work is reported in Nature Medicine with support from Kyverna Therapeutics.
Why it matters: For the small subset of rheumatoid arthritis patients whose disease has resisted up to eight prior biologic therapies, this is the first controlled-trial evidence that a single CAR T-cell infusion can produce sustained medication-free remission — though phase-2 head-to-head testing against existing B-cell drugs is still required before the therapy can move beyond experimental status.
