GAE Procedure Delivers Year-Long Knee Arthritis Pain

- Genicular artery embolization (GAE) using rapidly resorbable gelatin-based microspheres reduced knee osteoarthritis pain and improved function for at least 12 months in a 194-patient study published in Radiology.
- Dr. Florian Nima Fleckenstein and colleagues at Charité—Universitätsmedizin Berlin performed 239 procedures between July and November 2024, achieving a 100% technical success rate with no moderate or severe adverse events.
- Median pain scores on the Numeric Rating Scale dropped from 7 before treatment to 3 at both 6 and 12 months post-procedure, with 80% of participants exceeding the minimum clinically important difference threshold for pain relief at 12 months.
- Quality of life scores more than doubled (from 19 to 40) on the Knee Injury and Osteoarthritis Outcome Score, while daily activity scores rose from 53 to 71.5 and sports/recreation scores climbed from 15 to 36.
- Only 6.7% of participants experienced mild adverse reactions that resolved on their own, and follow-up retention remained high—79% of patients were still assessed at 12 months.
- The procedure targets abnormal blood vessels driving joint inflammation by injecting particles via catheter under fluoroscopic guidance, addressing a 'treatment gap' for patients who fail conservative care but aren't candidates for knee replacement, per the researchers.
- All 194 enrolled patients had failed at least three months of conservative treatment including physiotherapy, anti-inflammatory medications, and intra-articular injections; 45 patients (23%) received the procedure in both knees.
